Aspen Primary Care Network

BACKGROUND The Aspen Primary Care Network (PCN) represents the partnership between family physicians in Athabasca and Westlock. The Network Business Plan was developed over a period of time in 2008-09, based on need and demographics for the geographic area served, and works to achieve the Provincial objectives relative to primary care within our PCN. In total there are 19 participating physicians serving approximately 26,000 residents within the PCN region. The Aspen PCN became operational April 1, 2010. Since that time the Network focus has been on the development of strong multidisciplinary teams (physicians, nurses, allied health providers such as social workers, pharmacists, dietitians, etc.) in relationship with our partner providers in health care; Alberta Health Services, other human service agencies and local community resources, in order to address the priority initiatives of the PCN. Our priorities are facilitating health promotion and disease prevention, and successful self-management of chronic disease conditions.
